Please arrive 15 minutes early.
Please confirm your insurance requirements before the arrival of your appointment.
Any HMO plans that require additional documentation must be present at your appointment time. Failure to do this could result in a rescheduled appointment. (i.e., Referrals/authorizations)
Please also have your insurance card, ID, recent medical records that may pertain to your visit, any CT/MRI scans on a disk, lab results and any medications that you’re currently taking.
Appointments related to hearing, clogged sensation in ears, or ringing in the ears may require an audio exam with the MD visit. These appointments will need to be scheduled with our office directly to evaluate your concerns.
As part of our contract with the insurance companies, we are legally required to collect any co-pays or deductibles from you at the time of service. We ask that you be prepared to pay your copay at the time of check-in before being seen by our providers. Our staff will review your demographic and insurance information with you to ensure we maintain your correct information on file. This allows us to submit claims to your insurance carrier promptly.
A staff member will contact you by phone, email or text before your appointment to obtain any information needed for your appointment with the physician. If we are unable to complete this step, it may result in a rescheduled appointment.
